Community Programme

Cheshire and Merseyside Provider Collaborative’s Community Programme brings together Cheshire and Merseyside’s community providers, supporting each other to deliver the highest quality patient care.

As with all CMPC programmes, a key aim is to provide equal care to all patients, wherever they live in Cheshire and Merseyside.

By sharing best practice and creating economies of scale, through the programme, CMPC community trusts are able to use NHS resources in the best way possible.

Focusing both on urgent and planned community care, and in line with the NHS 10 Year Plan, the programme identifies opportunities to move healthcare ‘from Hospital to Community.’

In this way, it provides patients with effective alternatives to hospital-based services, enabling them to receive care in their own communities and, where possible, remain safe and well in their own homes.
