Sefton

Sefton is a diverse coastal borough of around 283,000 people, stretching from the industrial communities of Bootle and Litherland in the south to the seaside town of Southport in the north, with coastal communities including Crosby, Formby and Ainsdale.

Its coastline, beaches and green spaces are a defining feature, attracting millions of visitors each year.

Sefton also has significant health inequalities, with marked differences in health and life expectancy between communities, alongside significant levels of child poverty.

 

Primary care networks

At local level, health and care services are increasingly being delivered closer to where people live, through neighbourhood teams and partnerships.

Central to this are primary care networks (PCNs), which bring together GP practices and other health and care professionals to provide joined-up services for local communities.
