Cheshire East
Cheshire East is home to more than 425,000 people, across a diverse mix of market towns, villages and rural communities, including Crewe, Macclesfield, Congleton, Wilmslow, Nantwich, Sandbach and Knutsford.

Primary care networks
At local level, health and care services are increasingly being delivered closer to where people live, through neighbourhood teams and partnerships.
Central to this are primary care networks (PCNs), which bring together GP practices and other health and care professionals to provide joined-up services for local communities.
There are nine PCNs operating in Cheshire East:

- Congleton and Holmes Chapel (CHOC)
- Eaglebridge
- Grosvenor, Hungerford and Rope Green (GHR)
- Knutsford
- Macclesfield
- Middlewood Partnership

- Sandbach, Middlewich, Alsager, Scholar Green and Haslington (SMASH)
