Liverpool

Liverpool is a vibrant city of more than 500,000 people, with a relatively young population and a strong identity shaped by its maritime, cultural and sporting heritage.

Despite its many strengths, Liverpool has significant health inequalities, with life expectancy and healthy life expectancy well below the national average and substantial differences between neighbourhoods.

 

Primary care networks

At local level, health and care services are increasingly being delivered closer to where people live, through neighbourhood teams and partnerships.

Central to this are primary care networks (PCNs), which bring together GP practices and other health and care professionals to provide joined-up services for local communities.
